PAYMENT
Dr. Divakarla does NOT accept or participate in ANY insurance plans and is considered an "out of network" outpatient provider at Capital Family Psychiatry, PLLC.
This means that you are responsible for all payments for services at the time of the appointment.
Payment is accepted via cash, credit card (or debit and HSA/FSA cards), and checks only for established patients. We require that a credit card is kept on file via our secure electronic medical record system.
REIMBURSEMENT
A Super-bill can be provided at your request for you to submit to your insurance carrier. Reimbursement rates vary by individual carrier and plans.
